Aid for Mammography Form

We believe in the value of annual screenings for women and have seen the benefit and positive outcomes from early detection of breast cancer and other issues. We are passionate about these essential services for women and don't want cost to deter any woman from getting this essential care.

Our Aid for Mammography Fund helps cover these costs and is available to qualifying low-income, under-insured and uninsured Bitterroot Valley residents.

Apply for the Aid for Mammography Fund

To qualify for Aid for Mammography funds, you must be a Ravalli County resident and your gross family income must be equal to or below 200% of the federal poverty level.

  1. Apply: Please complete the Aid for Mammography Fund application.
  2. Approval: When approved, select one of our medical providers to schedule your clinical breast exam. When scheduling your appointment, please inform our scheduler that you're seeking care as part of the Aid for Mammography Fund program. This initial primary care provider appointment will be covered by the Aid for Mammography Fund, and they will assist you in scheduling the screening 3-D mammogram appointment with our Imaging Department.
  3. Appointment: During your appointment, ask your medical provider to submit a written authorization request for Aid for Mammography benefits. Authorization is processed quickly and on a case-by-case basis; qualified applicants will receive a breast screening services authorization letter. Upon receipt, you or your medical provider can schedule your mammogram screening appointment at our Imaging Department by calling 406.375.4888. Please inform them that you are approved for services under the Aid for Mammography Fund. You must present your authorization letter at each of your appointments.
